The mission of Medical Records is to maintain complete and accurate medical records, to safeguard all information against loss and use by unauthorized persons, to maintain medical records in a manner permitting prompt retrieval, and to assist TitanMED staff and patients as necessary.

The 21^st Century Cures Act is a federal law that promotes access and the free exchange of health information. It requires healthcare providers and payers to give patients access to electronic healthcare records without delay. The rule is designed to give patients and their healthcare providers secure access to health information.

In accordance with this act, Student Wellness has made Health Center records available to patients via their secure Titan Health Portal.

The following records are available to view and print from the Titan Health Portal:

• Signed progress notes
• Reviewed laboratory results
• Reviewed x-ray reports
• Immunization
• Optometry Notes
  
  

To request medical records:

- From January 1, 2023 forward, you may access a copy of your records via the Titan Health Portal, or via the instructions below.

- Prior to January 1, 2023, please follow the instructions below:

• Print, complete, and sign a copy of the Authorization for Release of Information Form . Fax to (657)278-3069

• Complete Authorization for Release of Information Form and email to studentwellness@ngskmc-eis.net
